Health Insurance from Assurant
For those who are ineligible for major medical insurance plans due to pre-existing health issues, Assurant offers KeyMed, a plan with benefits for prescriptions, office fees, lab fees, hospitalization fees, and surgery, as well as network discounts at no additional costs.
KeyMed offers coverage amount of $100,000 or $50,000
With the KeyMed $50,000 plan members receive:
- A $200 deductible, after which time your KeyMed plan covers 80% of costs
- Inpatient hospital benefits that cover up to $2000 per day for in-hospital care
- Office visits require a $25 copay for the first three visits while your KeyMed plan covers the rest.
- Outpatient medical services offers up to $500 of annual benefits
With the KeyMed $100,000 plan:
- A $500 deductible, after which time your KeyMed plan covers 80% of costs.
- Inpatient hospital benefits cover up to $4,000 per day for in-hospital care
- Office visits are covered is the same as what you would receive with the $5,000 plan, however, with the $100,000 plan, an amount of $250 worth of benefits is offered per person per calendar year
- Outpatient medical services offers up to $750 of annual benefits
The following benefits are covered the same on both KeyMed plans:
- Prescription drug coverage is offered in $15, $50, and $75 dollar co-payment amounts, and up to $250 worth of benefits annually.
- Emergency room benefits are $500 for two visits per calendar year. There is a $100 emergency room fee, but that fee is waived if patient is admitted to the hospital.
- Ambulance services apply to both ground and air ambulance services, which are covered at $100 for ground transport and $1000 for air transport .
- Surgical Services benefits extend up the scheduled amount with a $10,000 maximum benefit per condition
Specific Benefits offered by KeyMed plan:
- 12-month initial rate guarantee. Members lock in your premium rates for the first full year of coverage despite market fluctuations.
- Members get their choice of doctors and hospitals, as this is not a PPO plan.
- If members receive medical services within Assurant’s PHCS Healthy
Directions network, they are eligible for discounts on covered medical services.

KeyMed plan offers members who are admitted to the hospital for sickness and
certain dollar amount per day for treatments.
- Offers
members outpatient medical services, which extends coverage
to office visits, lab services, X-rays, and emergency room visits.
- Prescription
drug benefits extend to discounts on covered prescriptions,
and only $15 copayment for generic prescriptions.

Health Advocates Alliance Membership, an association dedicated
to the health and well-being of members, is available under the KeyMed plan
in all states. Access to a 24-hour nurse hotline, and a scholarship program
for qualified students studying a health-related field are available.
Enhance
your KeyMed plan coverage with extended dental and vision supplements
- Dental Insurance
is offered that can help offset the cost of routine and major dental services.
Members can choose between two different levels of coverage, and are free to
visit any dentist they choose.
- Dental
and Vision discount plan offers discounts to members on services rendered
by a nationwide network of dental and eyewear providers. Save up to 50%
on dental services and 60% on eyewear.
The
KeyMed plan is not a PPO plan, but members can receive discounts when using
doctors and hospitals in the PHCS Healthy Directions network. This can
